A COMPREHENSIVE INVISALIGN CASE WITH COMPLEX POSTERIOR TOOTH MOVEMENTS
The female patient, came with the loss of lower molars: tooth #35, tooth #46 and lost them more than ten years ago. The gap of tooth loss is large, causing shifting of adjacent teeth, tooth #46 is inclined mesially and lingually. Wisdom tooth #38 is incompletely erupted and mesially tipped, Wisdom tooth #48 is erupted.
The patient wishes to reduce bimaxillary protrusion, close all the gaps of tooth loss, and upright the molars. The patient only wants to treat with Invisalign, refuses TADs (minivis), refuses other appliances, does not want to extract any more teeth.
I treated this case with 2 rounds of Invisalign aligners, using only aligners and elastics (A-P and vertical correction). Difficult movements in molars (consisting of two wisdom teeth), including mesialization with bodily movement and molar uprighting in the buccal-lingual and mesio-distal planes, were very predictable.
